Correcting Mistakes
If you discover a mistake in the identifying information (incorrect spelling of your name, or incorrect date of birth or place of birth) in your U.S. passport, then you must apply for a new passport or passport card with the corrected information. We can no longer simply amend your current passport to reflect the correction. You will not be charged for the new passport. It takes about three to four weeks for the new passport to arrive.

Please submit:
- a completed Passport Re-Application Form DS-5504
- your current U.S. passport with the mistake in it
- two valid passport photographs
- proof of the information to be corrected: an identification showing the proper spelling of your name, or an original birth certificate showing the correct date or place of birth
